The following sections will support you to create an effective PBS plan, the person (if possible) and those involved in their care e.g. care staff and the person's family and/or representative are fully involved in the assessment process and co-produce and implement any approaches agreed on the plan. A good planning/tracking/reporting tool is the product checklist. This is easy to create, maintain and read and, most importantly, it is a good way to communicate with stakeholders that need this information.
